Assessment

       Geriatric assessments can provide useful information to individuals, family members, caregivers, and attorneys to provide insights about an older adult.  The assessment is completed in the comfort of the person’s home to evaluate the functional ability and safety of the individual. The assessment is strengths-based and utilizes humor to put the person at ease.

       Geriatric conditions are common and frequently unrecognized or inadequately addressed in older adults.  People may have a variety of  health conditions: cognitive impairment, delirium, incontinence, malnutrition, falls, gait disorders, pressure ulcers, sleep disorders, generalized fatigue, and dizziness which are collectively referred to as "geriatric syndrome."  These ailments are common in older adults, and they may have a major impact on quality of life. Geriatric syndrome can best be identified by a comprehensive assessment.

       Having decades of experience working with individuals with cognitive, mental health, functional and/or developmental disabilities, we can help explore safety concerns, discover issues and unravel mysteries.  Additionally, we have the capacity to set up a detailed care plan.  Our goals are to ensure safety and maximize the independence of the individual or couple, and reduce worry and concern of the family members or those closest to the person.
