Medical POA
A Medical Power of Attorney (MPOA) is a legal document that allows an individual to designate another person to make medical decisions when a person no longer is able to make decisions on their own. In other words, it names someone who is your voice expressing your choices regarding medical decisions. This could include expressing your wishes to doctors and medical staff regarding your choices including end of life wishes. The MPOA is sometimes referred to as Health Care Agent. A person need not be terminally ill, elderly, or facing a health crisis to execute a MPOA. Health care decisions include the power to consent, refuse consent or withdraw consent to any type of medical care, treatment, service or procedure. A MPOA is also sometimes referred to as health care proxy and health care power of attorney.
To be valid, the Health Care Power of Attorney must be in writing and signed by the individual seeking care. It has been our experience that seniors who need active Medical Power of Attorney services often receive care management when their health declines.

If you need an Elder Law Attorney to assist with creating your documents, you may wish to contact the National Academy of Elder Law Attorneys or the Colorado Estate Planning Attorney List for assistance in finding a lawyer who can best meet your needs.
